He was being scouted as a junior for football playing defensive end and tight end.

6'4" 225 is husky.
He is a large man.
He is strong and of healthy weight, not 110 pound sort of thing.

Had an older gentleman come in today for a med refill. Get in there, tell him he needs to give a urine sample. He says okay, then tells my assistant he'll need a urinal. Guy has a tough time getting around, so I figured he just use it when we were gone with the appointment. The dude stands up next to me, drops his pants to his ankles, whips his hog of of his baggy white briefs, and drains it right in front of me. At that point I said to myself "fuck this" and walked out

I thought you were an anesthesiologist?

What are you doing handling med refills and urinals?

Finished my anesthesia residency, now wrapping up a Chronic Pain Management fellowship. I'll be doing interventional pain management full time while anesthesia will be my side hustle

he is, he had to go to the Mayo clinic a couple of months ago to possibly go thru surgery again. they decided against it so now he is still just recovering and hoping to play football in 2019. He signed a letter of intent to play at Northwest Mizzou State. Possibly could have played D1 had he not had to sit out all senior year.

"Sciatica" is a common misnomer, even among primary care docs. If you have a radiculopathy (pain from back radiating down the legs) it could be from a number of things (discs, facet arthritic, degenerative changes, etc), although the pain itself is usually due to the nerve root being inflamed. Hence, the steroids (steroid is a potent anti-inflammatory, less inflammation ideally equates to less pain).

To your question, it depends on what is causing the "sciatica". Physical therapy, anti-inflammatory meds, epidural steroids are all conservative measures. If its from a stenosis, sometimes decompressive surgery is needed. Also, how long did the epidural last? We limit to 3 doses of steroid in any 6 month window, so I commonly have patients who regularly get 4-6 weeks of relief from the injection, deal with the pain for for a couple weeks, and get an injection every 8-10 weeks.

Without knowing someones medical history and the knitty gritty details, these questions generally suck because there are so many variables.

TLDR: It depends.

Doc hierarchy in the hoods for street cred:

Neurosurgeons>>>>>cosmetic surgeons>>>>>>>ortho surgeons>>>>>>>all other docs
