I had a patient today who watched porn, loudly, on his phone the entire time I was in there with him, about 5 to 10 minutes. I'd ask a question and he'd answer without taking his eyes off of it. Dudes sitting there with a full on rager in his hospital gown...so awkward

So long as I documented, I doubt Id get into trouble. As rude as I thought it was though, I shrugged it off easy enough. I deal with patients who spend my entire time with them staring at their phones. Obviously it goes without saying but I get the same treatment from high level execs when they invite us in to do a presentation. Its annoying but something you just get used to. The hospital Im at has a contract with the state for taking care of prisoners. Thus, anytime a prisoner needs surgery, we get them. Now, its not hard to imagine a prisoner stuck in a cell all day inflicting harm onto himself to get out for some time in the hospital. The beds are soft, they get their own room, they get a TV, etc. We get patients all the time who do stupid shit to get into the hospital, whether its swallowing razors/batteries, taking scissors to the abdomen, etc.
But the patient I had today took the cake so far for me. He ingested the long earpiece from a frame of glasses, but thats not the interesting part. This dude also took a paper clip, straightened it out best he could, and threw it up his urethra. His dick hole. Ive watched a prisoner literally pull part of his intestine out of his abdomen, and that didnt make me cringe nearly as much as seeing that shit today

Back in my ER days, some dude had just gotten arrested, comes to the ER for whatever reason...In the room, the instant the cops takes the cuffs off, he darts out. In a panic or hurry or whatever, he just ran into this open room that was 1 big room but had 4 ER beds, 2 on each side separated by a curtain. Two guards, prisoner remains shackled until they're asleep for the procedure, shackled back up before they wake up. The one time I got shaken up a bit was after I put a guy to sleep, went to place the breathing tube and saw that he had a razor tucked in his cheek. How the Fuck did that get missed?! We had a major incident recently with a prisoner beating the shit out of their guard once the shackles came off. They were inpatient and he used the serving utensils and tray and just pounded on the guard. Scary shit.

Yesterday was brutal and seemed like it would never end, but at least I got a good story out of one of the cases.
First a little background. In general anesthesia, patients are put to sleep and a breathing tube is placed in the airway. Through this, gas agent is given to keep the patient anesthetized, while also controlling their breathing. If a patient is deeply anesthetized, or paralyzed for the procedure, the ventilator machine will control the patients ventilation. There are a lot of factors that go into it, but for simplicity sake, know that at times a patient will try to breath over the machine. Most of the time this isnt an issue, but occasionally the patient will 'buck' a bit on the tube, and may appear as though the patient is trying to cough (cant really think of a better descriptor at the moment). The patient moves a bit, adjustments are made to the anesthesia, and the surgery is back on its way. Doesnt happen often, but it does happen.
One of the surgeons we have is the local prick. I understand surgeons have stressful jobs and can get edgy, particularly in high stress cases. I get it. However, this guy is a genuine asshole for no good reason, and most in the hospital know it. On top of it, he is extremely demanding of his anesthesia providers, and honestly unreasonably so. Many surgeries dont require paralysis, and as such we dont give the muscle relaxants that can have a slew of side effects with them. Because of this, we try to run the gas a little higher to keep the patient deeper in an effort to minimize patient response to the painful stimuli. However some patients, despite high levels of gas, can still respond with some minor movement. The vast majority of surgeons understand this and can work with us if the patient has some minimal movement, we give more meds and we're on our way. This guy flips his shit at any movement and is usually pretty boisterous about it.
So yesterday we had a scheduled hemorrhoidectomy, aka removing hemorrhoids, on a guy in his 50's. The positioning is such that the patient is put in a lithotomy position, with the surgeon standing between the legs:

So the procedure is trucking along just fine, Im running the gas pretty light because the patient had some other health issues I was taking into account. About halfway through the case the patient 'bucked' on the ventilator and lets go of some explosive diarrhea all over the surgeon. Surgeon was bending over a bit too, so a LOT of shit got into his face (he was wearing mask/eyewear though). It took everything I had not to lose it, particularly when he stepped aside and you could see a vague silhouette of him against the wall. I though he was going to go off, but surprisingly he kept it together and shrugged it off. The entire staff was walking on glass the rest of the case
